What the Service Offers

Social Support Group operates in the Active Lifestyle Centre and provides activities to frail older people, designed to develop, maintain or support the capacity for independent living and social interaction, which are conducted in a day centre setting.

Active Lifestyle Centre includes group excursions / activities conducted by centre staff, in and out of the centre.

These services involve a structured program and may include such activities as:

  • music therapy
  • allied health care
  • outings and day trip
  • personal care (e.g. assistance with toileting)
  • provision of a meal and assistance with eating
  • provision of transport to and from centre-based care
  • communication with carers where appropriate

Carers, if any, will receive a break from their usual caring relationship.

Social Outings

If you are becoming house bound and tired of watching television etc, we hold on average of two social outings on Mondays, each month. They are usually held on a Monday. We pick you up from home and drop you home when the outing is over.

You will be asked questions over the phone to help work out your needs and care arrangements – this takes at least ten minutes.

You will need your Medicare card.

If you would like someone to call My Aged Care for you, you will need to give them your consent.

If you’re calling for someone else, they will need to give their consent.

Have a face to face assessment

My Aged Care may arrange for a trained assessor to come to your home. With your consent they will assess your care needs and eligibility for services and work with you to develop a support plan which addresses your needs, goals and preferences.
